UCI Health Insurance
All UC Irvine Extension students are required to have health insurance. All students are automatically charged for UCI health insurance, unless they have health insurance of their own and submit a Health Insurance Waiver. If you wish to waive UCI health insurance, please complete the Health Insurance Waiver below and submit it to the ESL Office or Cashier's Window before your fees are due.

How do I get my medical bills paid?
Make a copy of all of your bills and receipts and write your group policy number (US002643) on all the original itemized bills. Have all your itemized bills attached when you send them. All itemized bills and receipts should be sent to:
Personal Insurance Administrators, Inc.

Agoura Hills, CA 91376-6040
If you have any questions about the status of your claim after it has been submitted, please call Personal Insurance Administrators, Inc. at 1-800-468-4343.
A claim must be submitted within 90 days after an Injury or Sickness has occurred in order for the claim to be paid. Please always keep a copy of all documents submitted for claims!
